disease, or Best vitelliform macular dystrophy, is an inherited eye condition affecting the macula, the central region vital for clear vision. Typically
appearing in childhood or adolescence and progressing over time, it results in
central vision loss. Named after German ophthalmologist Dr. Friedrich Best, who observed a family with various stages of the disease in 1905. Best disease can reveal a distinctive fried egg appearance in the
macula. This look, resembling a yellowish egg yolk, is caused by the buildup
of lipofuscin, a fatty substance, in the retinal pigment epithelium cells.
These cells support the retina, the light-sensitive layer of tissue in the eye.
Imaging techniques, like fundoscopy, allow us to see this fried egg appearance.

Reference

Tripathy K, Salini B. <span style="font-family: "times new roman" , "serif"; font-size: 12pt;">The
scrotum ‘houses’ the testes and helps to keep them at a lower temperature than that
of the rest of the body because sperm production takes place best at cooler
temperatures.

<span style="font-family: "times new roman" , "serif"; font-size: 12pt;">In
a rare disease affecting the scrotum’s skin, nodules of calcium accumulate within it - scrotal calcinosis - and because of their raised mottled look the nodules give the scrotum an
appearance not unlike a strawberry in some cases – strawberry scrotum.

<span style="font-family: "times new roman" , "serif"; font-size: 12pt;">Scrotal calcinosis is not life threatening, its <span style="color: blue; font-family: "times new roman" , serif; font-size: 16px;">underlying cause <span style="color: blue; font-family: "times new roman" , serif; font-size: 12pt;">is
unknown. Affected men usually between 20 and 40 years have normal measurements
of calcium in their blood. Treatment for scrotal calcinosis often for cosmetic
reasons is by surgical removal of the affected scrotal region.

Under the light microscope in some cells of anaplastic large-cell
lymphoma (ALCL) the nucleus is ring-shaped with a ‘central opening’ thus resembling
a doughnut in look – doughnut cell.

ALCL is a rare cancer of the lymphocytes (the T-lymphocyte more precisely),
a kind of white blood cell, thus the designation lymphoma. Anaplastic refers to
cells that are abnormal in shape and function and large-cell pertains to the
relatively big cells of this cancer.

Depending on individual circumstances treatment for ALCL can
include, radiotherapy, chemotherapy, surgery, etc.

Under the light microscope
in classic cases of the soft tissue cancerous tumor fibrosarcoma, the cells are
arranged like the way the branching spines of a herring skeleton are arranged –

An

such columns.

Brenner tumor (named after the German
physician Fritz Brenner 1877 – 1969) is a rare ovarian tumor apparently derived
from the surface epithelium – outer lining - of the ovary. The tumor is usually
benign (not cancerous), but can occasionally be malignant (cancerous). Under
the light microscope, the nuclei of Brenner tumor cells may have a longitudinal
groove similar in look to the longitudinal groove of coffee beans – coffee bean nuclei.

forebrain also known as the prosencephalon normally separates into two halves
(cerebral hemispheres) during the development of the unborn baby. Failure of
the prosencephalon to separate leads to the rare disease, holoprosencephaly
(HPE). The prosencephalon may fail to separate to different extents; where
failure to separate is complete, alobar HPE results, which is the most severe
form of HPE. The brain in alobar HPE resembles a pancake – pancake brain.
<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Salt and pepper retinopathy refers to tiny light specks (hypopigmentation) interspersed among tiny dark specks (hyperpigmentation) that may be seen in those affected by the rubella virus when the eye’s back (retina) is viewed using special equipment.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">These specks resemble salt and pepper in appearance – salt and pepper retinopathy. Besides rubella virus infection, syphilis and the toxic effects of some drugs can produce salt and pepper retinopathy. Salt and pepper retinopathy normally does not affect vision.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Vaccination of both males and females against the rubella virus helps prevent harmful effects of the rubella virus on the baby such as blindness, deafness, heart defects and other dreadful manifestations of the congenital rubella syndrome.

Headache, vomiting and weakness can be the initial
manifestations of medulloblastoma, the most common malignant brain tumor of
childhood. The nuclei of some medulloblastomas when seen under the microscope can in shape remind you of carrots
– carrot-shaped nuclei.

Medulloblastomas affect the cerebellum, a part of the
brain concerned with fine-tuning movements but not initiating movements. At
times medulloblastoma cells may spread to other regions via the cerebrospinal
fluid, which is a clear colorless fluid surrounding the brain and spinal cord.

Surgery, radiotherapy and chemotherapy are forms of treatment for medulloblastoma.

The inferior vena cava (IVC) returns blood from the lower body back to the heart. In the tummy, the aorta and IVC lie side by side; the aorta lies on the IVC’s left side.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">One of the aorta’s tummy branches is the superior mesenteric artery (SMA) which supplies the intestines with blood.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Blood returning from the left kidney (within the left renal vein) must cross in front of the aorta to reach the IVC because of the relationship between the aorta and IVC - the aorta lies on the IVC’s left side.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Rarely, the left renal vein can become compressed between the SMA and aorta causing it to dilate. <span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">In the rare inherited disease – Wilson’s disease – named after a doctor who described it, copper accumulates abnormally mainly in the liver and brain. <span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Treatment for Wilson’s disease includes avoiding foods with copper, using medications which prevent absorption of dietary copper or bind to copper within the body and liver transplantation.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Leucine is one of the amino acids, which are the building blocks of proteins. Leucine is an essential amino acid meaning that the body cannot make leucine from scratch; it has to be obtained from the diet.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt; mso-bidi-font-family: Verdana;">Isovaleric acidemia is a rare inherited disease in which there is a deficiency of the enzyme responsible for the processing of dietary leucine. Failure to process leucine results in the abnormal accumulation of isovaleric acid and other compounds, resulting in an affected individual having a body odor reminiscent of ripe cheese or sweaty feet/socks.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">The heart is one of the first organs to function; it begins to beat from about 5 weeks of pregnancy. Normally after birth, 4 pulmonary veins return oxygenated blood from the lungs to the heart’s left side. This blood is then pumped by the heart to the rest of the body.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">During pregnancy, when the future baby’s heart is developing, abnormalities in the heart’s development can occur. One of these abnormalities is total anomalous pulmonary venous connection/drainage (TAPVC) where the 4 pulmonary veins do not drain into the heart’s left side.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Patients with TAPVC because of the abnormal internal anatomy may on chest X-ray have an appearance akin to a cottage loaf – cottage loaf sign.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">The middle of the face around the nose area may be hypoplastic (underdeveloped) in Larsen syndrome giving the face a look resembling a dish – dish-face. Other features of this syndrome named after Larsen who described it can include a tendency for the large joints to dislocate as well as abnormalities of the hands and feet.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Larsen syndrome is rare and is typically inherited in a dominant manner; meaning only one copy of the abnormal gene is enough to cause the syndrome when inherited from a parent who has the syndrome.

<span style="font-family: "Verdana","sans-serif"; font-size: 10pt;">Mid-face fractures can also result in one developing the dish face-deformity.
